Call GW using the number on the website in the upper right...
They WILL send you a new instruction sheet.

I don't see how age would enter into it. You are a consumer, their product had a defect.
Did you buy it from a GW store? (should not matter, but I ask for completeness).. and.. do you still have the box? They will want to know some codes off it.
I am serious with this.. I have NEVER had a shoddy experience with them.. and the ones that I have had, make me PRAY that I get packing mistakes.
To expand..
I bought a box of the snazzy newish Wolf Guard Terminators, and the sprue with the shoulderpads was mis-molded. I was missing *4* of the 13 pads in the box. I called them, told them the problem, and gave them the codes from the box back. Guy on the phone told me he would send replacements.
I got a whole new box. Not the one sprue, ALL of them.. DANG!
Ditto happened 2 years back with the Loota/Burna box I bought.. mis molded body sprue.. BANG .. got a new box.
I DOUBT and don't want to raise your hopes.. but I am 99% certain that you will get the instructions.. althought I suspect you will NOT get a new Monolith.. lol
EDIT .. Arschbombe probably has it right.. I have not ever bought a Monolith, so he has better info than I..

They won't just send an instruction book. Usually if something is missing from a box, they'll send the whole box again. So if it turns out that you're missing your instruction booklet (assuming there even was one), then calling Cust. Service might be worthwhile, as you -might- end up getting a free monolith.

The normal Painting and Modelling forum is probably a better place for this kind of question.
However you got the scan of the instructions from Arschbombe...
So it has all ended well.
