Funny enough, when a girl is actually a boy, there's very little evidence of it and it's usually a surprise (both audience and usually the cast don't know about it), but when a girl is pretending to be a guy the audience spots it right away but the cast remains oblivious.

It's all a huge embarrassment to people who actually have more than a passing interest in these things.
It's not that anime is some underground secret thing, but things that are related to it are, and people inevitably find out about these things by connecting the dots and ruin them for people who's entire lives revolve around them, who live off the happiness it gives them.
