Apparently, VX was also used in Japan by some guys who managed to mix it up in their homes - they attacked three people, one died. It's possible, but difficult, I'd assume.

Nevertheless, it seems more probable that this is professional stuff, not some home-cooked variant from some looneys. The whole attack seems too well planned to leave it to some haphazardly brewed concoction.
