If its not level, its cocked. Never really had an issue deciding when one is cocked, as long as you're consistent then the averages will turn out the same anyway so...

I've had that happen on a few occasions and I ALWAYS take the topmost side on both dice. They both landed with one side obviously facing up, so i see no reason not to.

I look at it this way. If the dice was cocked on an ice cube, and that ice cube melted, whatever the number it would've landed on is what you use.

I did have a mate drop 2 sixes on either side of a blast template the other week, they were only slightly cocked but we both knew what we had agreed to so there were no major dramas.
Saying that someone once asked me to reroll a dice that was on top of a army list, just a single sheet of a4, you're taking the piss with that one.
But, "flat on the table" means "flat on the table." I'd ask you tor reroll it, too (regardless of the die roll).
That's the problem with such a limited scope of acceptability. it can easily get out of hand.
What kind of terrain do you use? In my group, it's often near impossible for a die to land flat on the table. Some of the terrain is GW RoB and some is professional made terrain from Italy. There is little flat space on the RoB and almost none on the italian stuff.
Not trying to nit-pick your rules. Your rules are your business. I'm just trying to show the possible annoyances that can come from something so restrictive.

I use the terrain either at warhammer world or Maelstrom games, its the only two places I play. They generally have 25% of cover so there are plenty of places to roll even large numbers of dice.

I have two methods really. If I have a dice tray everything goes in the tray (exceptions are deviation dice with are easier close to the point of deviation).
If I do not have a tray, then I re-roll anything that does not land flat on the table. If it lands on terrain, paper, books ect...it gets re-rolled no matter how flat it is. I am extremely consistent on this and many of times I have picked up 6's that were flat but on terrrain and re-rolled them and got 1's

As long as my opponent plays it consistently the entire game, I'm not worried about what he counts as "cocked" or not. Different folks have different standards... you just have to be consistent.
