So, hold up just a fething second. Perturabo was losing a battle against the Retribution fleet, the Imperial Fists would have had overwhelming force from the 2nd wave hitting Pert's ship to assassinate him, and Dorn's massive insecurities and gak timing ruined a chance for the IWs to be effectively eliminated from the Horus Heresy.
Are there any massive errors on the Loyalist side that aren't Dorn's fault?

Its not Dorns fault is it, he had no idea that his order would have that effect. What we learn is that the fists will follow orders explicitly, even when it's tactically a terrible option.

Shadows of Betrayal, first story. The second is Curze reflecting on Dorn's hissy fit about Curze killing prisoners and the subsequent beating, imprisonment, and the destruction of Nostramo. The third is Dorn pissing his pants thinking about Curze.

it's all well and good to say thatw ith your omnipresent view, but war time is a time of chaos, orders are issued for a reason, and a LOT of the time you need to trust the guy issuing em knows more then you do and is issueing them for a reason. sure oppertunities are lost, but sometimes disasters are avoided
