So I'm in the process of tuning my Word Bearers and I am buying Chaos Cultists. Now being fussy like I am about theme in my army I was wondering if anyone knows about realistic alternatives. So instead of the slight Mad Max theme, something closer to insurgents or even just civilians with guns. My vision for them would be a levy of fanatics that my Word Bearers drummed up out of a local population. Something like people you knew as friends as neighbors taking up arms and siding with the Word Bearers. Sorry if that sounds kinda weird but if you know of any modernish looking models that fit that bill.

Anvil Industries has a new range of modern-looking militias, with hoodies and other civilian clothing. Otherwise you could use Genestealer cult Hybrids? The worker ones?

You gotta remember, these are 40k civilians. So there’s not really many guys in jeans and T-shirt’s or women in skirts living in suburbs, at least in the major human worlds. It’s grimy overworked wretched people with rebreathers so they don’t die from air pollution and bulky jumpsuits to protect them from violence and environmental hazards, and weird hair or tattoos just to stand out in some tiny way in an awful uncaring world as they slowly go insane from the relentless imperial dogma and threat of annihilation in so many ways.
There probably are cleaner, happier people in the top echelons of human society, with fewer problems that might resemble our own societies, but they’re also a lot less likely to become violent cultists.

Well that would be the general case for a hive world.
but not all worlds are like that.
Agricultural worlds would probably be simple astra Militarum patterned overalls and t shirts a few civ patterened rhinos with tractor parts.

Plenty of good advice above. +1 on Victoria, Anvil and the Frostgrave cultists.
Maelstroms Edge released four sprues of armed scruffy civilians the other week. "The Broken". They look good, are simple fun to build/mod and come with loads of bits and alternatives. As a bonus they are realistic in the way that you can build them as mixed squads of men and women.

I put together a list of alternates a few years ago.

One of the big challenges to alternate sculpts is they tend to be expensive - as much as $8 per model for really good ones.
That said, if you are willing to be patient, the usual rules around secondhand models still apply. I did pick up a ton of Pig Iron / Victoria Miniatures models through trading forums and eBay for a lot less than they retail. You can strip paint from them the same way you can any other model.

The Dark Vengeance cultists are exactly what you are asking for: 40K civilians with guns, as are the hybrid neophyte GSC cultists.
I used DV cultists, Empire Flagellants, GW and Mantic zombies and various heads from Maxmini and others for my Nurgle cultists. Not exactly the easiest figs to modify, but it can be done. https://www.dakkadakka.com/dakkaforum/posts/list/483650.page
