Odd Taphrian(?) Feral Orc concept. Since fantasy so often is about taking real life inspiration and run with it one step further, you know this is destined to happen with Orcs (if their reproduction is flesh not fungus). Phallic sheath based on those of e.g. Zulu, Somba and distant Papuan koteka gourds. Made into stone-tipped spear.

Thats 40K. It is left ambiguous for fantasy but 40K spawning method was kicked into overdrive so the faction could keep up as a major threat.

Nope, in fantasy they reproduce by spores too. In the End Times a Fire mage wich name I can't remember was exterminating the greenskins in the badlans with his fire based magic, making them unable to reproduce because he was burning the spores.
Only in bloodbowl (Is own universe) do you have orc females.

That was a major retcon from earlier editions where there were not just passing mentions of female orcs but also half-orcs (which implies more or less matching genitalia).
Besides orc cheerleaders there was also a very female orc mini with a rolling pin. Maybe others.
So orcs were mammals until at some point someone decided to insert the spore thing into WHF. Probably because it was very much GW proprietary.

I think in Humans it also serves a display function, but what I understood from Cheesecat's comment was that it is not just a reproductive organ.
