Incorrect. Only 1 out of every 5 Ardboyz can carry either a banner
OR a glyph. You can't have both in the same 5 model group. To get both you'd need a group of 10.
From the warscroll;
"STANDARD BEARERS: 1 in every 5 models in this unit can either be a Gorkamorka Banner Bearer or a Gorkamorka Glyph Bearer" Note the OR I highlighted in red.
So in a 5 man squad;
2 shields
1 drummer
1 banner
OR glyph
- you gotta pick the effect you think will work best for you. Personally I'd pick the banner. Now in a 10 man squad I'd have one of each...

Yes, it has to be on the shield bearer that failed the save. Because you have to specifically assign a wound to the model in order to roll the save.

Roll a dice each time you allocate a wound
to a model carrying an Orruk-forged Shield.
On a 6, that wound is negated."
