Are there any stories of an unforgiven chapter falling to chaos? I thought that would be an extra interesting story as they went over to chaos they would take all the dark angels dirty secrets with them, potentially broadcasting them to the inquisition etc

None that I'm aware of.

That said, the idea that they've kept their secret from everyone in the incredibly nosey and prying Imperium, for ten thousand years, requires a certain amount of plot protection and a lot of willing suspension of disbelief.

I like to think that Dark Angel's situation is like that of Captain Shakespeare in Stardust; the Dark Angels will go to any lengths to hide their terrible secret from the Imperium, while meanwhile everyone in the Imperium already knows the secret, but they pretend to be ignorant out of respect.

Not so much out of respect as indifference. Pretty much every chapter has had companies or entire successors turn to chaos.
It just isn't the big deal that the dark angels think it is.

Its literally why SM were divided up into Chapters in the first place- so when it does happen, it isn't a major conflict.

Even though the Badab War proved that multiple chapters going bad at the same time was still a problem and could happen...

Sure but if you were the chapter master of an unforgiven chapter and you turned you back on the DA the first thing you’d do is send what even counts as an email in 40K to the inquisition blowing the whistle on the DA secrets

It was still smaller problem than Huron converting whole legion into renegades. Which he would easily do seeing he was unusually brilliant and charismatic leader so he would rise to command one if he could.

Also, Codex worked seeing Huron could drag 3 other chapters with him into renegacy but despite owing him a ton of blood debts, these 3 chapters remained independent of him and only followed him halfheartedly then quickly surrendered. If that is not a ringing endorsement of the whole system (and proof how stupid and short sighted Dorn was) I don't know what is...

Well, there are DA successors who think the whole 'unforgiven' business is idiocy and don't want anything to do with DA anymore, but none of them appears to have talked. Maybe they kept quiet out of respect (or anyone in the know was assassinated by DA) but it happens.

TBF after being beaten into a pulp by the Minotaurs until there are only 300 of you left and most of them are wounded, surrender was kind of the only option for the poor Lamenters.
