Units with 7+ models must be in coherency of 2 other models vs 1 for smaller units.

You can pile in non-BTB models, but must be in that coherency afterwards or you can't pile in.

You can consolidate non-BTB models (either towards the enemy or the nearest objective) but must end in that coherency or you can't consolidate.

You lose a big base character and that fight could really grind down to a practical halt because nobody can consolidate or pile in with that one loner in BTB contact on the side of the fight away from the objective.

Lets say Warboss Sneaky Git, and Captain Everymarine square off and through rules (still fight after death, exploding/Deadly Demise, whatever) they end up killing each other. Both factions (for the purpose of this example) now have 1 model out of 10 (20 for the orks) out on his lonesome only in coherency with one guy. Because of the base size of those two characters and BTB nature of the surrounding fighters neither side can consolidate across the empty space to re-establish conherency? I mean the semi-obvious choice is to remove that loner as a casualty as soon as possible - but if its another character, or a special equipment guy that would suck, plus maybe it works out in your favor not to as the other side is also paralyzed from piling in their 15 models (I'm exaggerating for effect - this would vary from situation to situation) that ended just short of engagement range.

I feel like one or the other of these moves should allow BTB to move- i.e during pile-in, or (probably) consolidation you can slide BTB models around to recreate coherency. Or am I missing something else in the main rules like that loner dude is just killed for not being in coherency?

I just see various scenarios where a bunch of the models on both sides can't consolidate/pile-in to a fight because they can't maintain 2+ model coherency and thus 18 guys stand around watching 4 guys fight.

At the end of a turn you kill models that aren't in coherency (page 6). That should solve the issue, no? You would have killed these loners before the next pile-in can start.

This seems like a theoretical problem at best. Killing a leader is not that easy to begin with (you need precision), both killing each other is very unlikely. If you set up your unit coherency in a way to make a large based model your lynchpin without putting a second model in as backup, you just have to accept that being greedy has its consequences, lose your pile-in and remove the loner as casualty.

Just like in 9th, I expect the only people hit be the coherency casualties are those playing with fire by stretching a unit further than they should. If someone had to kill a valuable model because of it, it was always because of conscious risky choice or because of making mistakes when removing previous casualties. The most common one being that they somehow wanted to keep both a sergeant and the heavy weapon guy as last models despite them being 5" apart.

7 Ork facts people always get wrong:

Yeah, this seems like an extremely niche example of a self-inflicted problem. There are rules for removing models not in coherency anyway, so at worst this is a one-turn "problem". I'm not really convinced it's a problem in the first place - just a potential oddity if it ever does happen.

It is a shame they didn't have a rule like that in 9th edition.
