In the beginning, when I and those of my friends who decided to pick it up were quite clueless indeed.
WS4 against WS6... To hit is like to wound, right? Right! So boyz will hit a captain on a 6+ while he hits back on a 2+
The amount of situations we now can look back on and laugh about is innumerable.

I had Ahriman turn into a Chaos Spawn and then suffer a few wounds from a predator tank. So, I sent the Spawn to charge the tank who was next to a unit of Thousand Sons holding an objective. The tank explodes, taking the last wound from my Warlord Spawn (giving Slay the Warlord) and wiping the four Thousand Sons off the objective.
I lost the game and was sad.

First games with Crons.....Scarab has 4 attacks (5 on the charge), 4 scarabs per base, so each BASE gets 20 attacks on the charge (and each base had 12 wounds - three for each of my four scarabs).
My scarabs were unstoppable.
Then I read more closely :(

I had the same problem. Me and someone else did not know how the point system worked so our armies were all the armies we put together at that point. This meant what I had for a space marine army a tactical squad, captain, devestator squad, and an attack bike while my opponent had most of the black reach orks. Didn't help matters that that I forgot to shoot in the shooting phase resulting in my entire force being destroyed on the first turn.

Adjusting to the idea of Rapid Fire weapons after playing Eldar for so long who have none. Realizing that I'm going to get DESTROYED because I failed to initiate a charge due to firing my Bolters on my Space Marines.

I've deployed in range of my opponent who has first turn and didn't pay attention to the LoS blocking terrain (he could still see a good chunk of my units). I ended up losing 1/3 of my army in turn 1.

Not me but a Mech IG player I played against. He decided to put his entire army on top of a Landing Pad, for the 4++. My Mawloc cost him the game with a perfect DS over his entire army. Greebo had spent an irritating two minutes in that box. Technically, a cat locked in a box may be alive or it may be dead. You never know until you look. In fact, the mere act of opening the box will determine the state of the cat, although in this case there were three determinate states the cat could be in: these being Alive, Dead, and Bloody Furious.
Orks always ride in single file to hide their strength and numbers.

Emperor's Will (I think... The one with two objectives).
I had the game in the bag. It's turn seven and I'm going second. I'm contesting my opponent's objective and holding my own with two Assault Marines (Blood Angels), one of whom has a meltagun. I have linebreaker, but he has first blood and warlord. So, basically I'm going to win 4-2.
Except, I'm an idiot.
Close by my objective there's a drop pod. Can't contest it, but hey, I have a meltagun, and for the hell of it, I shoot it. I hit. I penetrate. It explodes. Meltagun dude dies. Other dude fails his leadership. Runs off the table.
I lose 1-2.

Hah! Reminds me of the Only War game I played in the other day.
Three of us, Sarge (me), Heavy Gunner, and Operator. Whole thing starts going downhill when we decide, screw it lets jump in on grav-chutes. Heavy Gunner fails the roll, losing every wound, and breaking his leg. After getting a splint on it we continue to investigate a comm bunker. While investigating, some Orks in a Trukk roll up. The Gunner goes to the roof where there was a Heavy Bolter with attached Grenade Launcher. The Operator rigs up a frag grenade booby trap and then joins me behind a bit of cover. The Orks ride up and half of them disembark. The Gunner pelts the Trukk with grenades with little luck. Operator lobs a grenade into the first group killing 4 with the last 2 dying from the trap. Rest of the Orks start to pile in and after a couple of rounds kill the Operator. After (finally) dispatching the rest of the Orks, I tell the Gunner to blow up the Trukk. Well, he does. And we forget that the Gunner has no wounds left. The explosion (critically) hits him in the arm causing all of his ammunition (grenades, heavy bolter rounds, etc) to explode. I survived. Lost both men, but man was that death worth it.

First game using jump infantry and one of the first games using MC, I didn't know MC had power weapons and that jump infantry can't get pinned
My Gargoyles sat useless in the corner, my Tervigon was murdered by Howling Banshees, Tyrannofex was killed by Striking Scorpions, and Hive Tyrant was killed by Dire Avengers after killing a squad..
My Ymgarls killed 3 (almost 4) units of troops.
We were playing Kill Points (I refuse to play this against the Tau player in my group) and my opponent got twice the points I got, I got 4 or 5 points he got 9. feth Kill Points. Yesterday. Playing Relic. 2k Marines vs 2k Necrons. The relic got placed in the middle of the table as required. Putting it behind a LOS blocking piece of terrain and just beside a hill. Marines go first.. During deployment I scout my scouts with sniper rifles into a spot where I lose partial sight on some of his big blob of warriors. So what do I do? Drop my drop pod down to block the only path to the relic. Well, perfect hit!!!! My Sternguard proceed to use combi meltas and blow his monolith back to the tomb world it spawned from. While my scouts....well they sit on their butts cause now I cant see my opponents army at all... there went that brilliant idea :(

Playing a game with a dude, tabling him at turn 3, feeling bad about it so giving him some pointers about how to beat my list, then he says "You were playing 1850?"
Turns out he had a 1500 point list, and neither of us thought to ask about it before the game.

5th edition chaos, rolling on the dreadnaught crazed table and getting the shoot twice result with its plasma cannon and being so excited i resolve it then and forget to move my entire assault army, happened turn 2 and turn 3, cost me the game. Le stupid.

Oh boy....yeah. I get the "got excited..." thing....ouch.
More than once I got too carried away with some plans, and skipped movement and started to declare shooting.
Another game, I had some kroot (painted green) on some green terrain that were supposed to capture an objective.....I did not see them and forgot....it was not until the end of my turn until I noticed...
lost by one objective.
I have had several games where i have forgot about reserves, powers or even worse, left models in my case .....the shame.

And the game where a certain someone added an extra weapon to their Dreadnought
I must admit I do forget that Veterans of the Long War does actually have a purpose other than giving better leadership, especially when fighting Space Marines

Not my mistake, but someone decided to set up a three table apocalypse battle with a greater daemon on one table protected by two shield generators on the other tables.
Of course, nobody told the players that was how it was going to work until after deployment, so you ended up with this greater daemon facing down a literal parking lot of leman russ and shadowsword variants.
