I'm in my second year of being a doctor and a registrar's position is opening up in OBGYN here at the Royal Brisbane Women's Hospital (the largest tertiary centre north of Sydney).
Essentially it will mean I have my own resident to get to do the lame resident type jobs I don't want to do but there is a huge jump in responsibilty and knowledge level and I'll have to crack the study like nobody's business.
My plan was to do the registrar's job next year and then become a training reg the year after that. But with the opportunity to do it now it could move me a year ahead of scedule.
As far as I can tell there is only one other doctor who will be going for it (she did her OBGYN rotation before me so is only 5 weeks ahead time wise but as I've been stuck in after hours roster i havn't had a chance to hob nob with the bosses)
However I did it last year and had to do the after hours roster then (normally never done by an intern) and have 9 years of nursing behind me.
The other big con is that I haven't applied for a job via interview since I had to do an interview to get into medicine in 2002. GLEEP!
It also means that ALL gaming and painting will be put on hold for at least 3 months until I get up to speed. :(
Any tips for interviews?
I know the people who will be doing the interview.

I was in a similar position back in january, applying for psychiatry training rotations having not had an interview since getting into med school. We had a half day session at work about applying for jobs, most of which was BS but did have some useful interview practice, basically medical interviews appear to be a case of answering any question with a slightly unrelated reitteration of your skills, so you want to appear
A) confident in your abilities and knowledge.
B) aware of your limitations and when to ask for help (at the end of the day they don't want you putting their neck on the line)
C) keen to learn more to address B
D) dead keen on doing research/audits/clinical governace stuff.
and also we had lots of emphasis on reflective practice etc...
You need to show a long term dedication to the specialty etc and read between the lines of any question, so if asked about a mistake, talk about something which casts you in a good light and show how you learned from it. mention audit a lot.
Sounds like you already know about the hospital/training but asking around and showing interest will help.
Brown nose your arse off, A guy I work with came to ITU handover rounds after his shifts finished for a month just so his face would be recognised when it came to applying for a job.
For interview practice ask your boss's to do mock interviews and give you feedback, or apply for posts you don't really want and use them as practice too.
Hope that makes some sense. Probably most of that isn't relevent cos Oz isn't going through the same retardification that the UK is in terms of medical training, just ask any of the brits you work with.

All good advice, particularly the last bit there. Nothing preps you for the real thing like the real thing...sort of. You don't even have to apply for positions in the hospital. One at your local fast food joint might be almost as good if you just want ot bursh up on your inteview skills.

Got to do a vacuum removal of a baby the other day.
Imagine having a baby's head on view, stuck, using a giant needle to inject anaesthetic in the pudendal nerves (yes via the vagina), attatch a giant suction cup to the baby's head, pump to decrease pressure and therefore increase suction, apply traction, baby moves but stuck again, grab scissors and perform an epesiotomy, traction again, baby head out, unwrap the umbilical cord from the baby's neck, next contraction remove baby and put on mum's chest, tripple clamp and cut the cord, take arterial and blood gases from the cord, traction to cord whist guarding the uterus and remove placenta.
Start to finish 4 minutes, including 2 minutes to wait for the pudendal nerve block to work.
Then I had the exciting job of repairing the epesiotomy... 20 mins. :(
Nursing was much more easy going...

I'm assuming it was live delivery, because otherwise why would you be concerned about the cord around the neck?

I can't believe you can tolerate that field. Of all the fields of medicine I would find fascinating, Gynecology, Urology, Proctology, and anything to do with the throat or nose would not be on the top of my list. Thank god we have you to do these kind of things

Yes live term delivery.
Episiotomy: (They are invariably very similar, just the number of deep suture layers change, I err on the side of caution and do more when needed)
2-0 vicryl rapide suture material.
Intravaginal apex secured.
Continuous sutures to hymal remnant.
x4 layers of deep sutures to perineal muscle/deep tissue.
Subcuticular sutures to skin.
Tied off at hymal remnant.
No pics.
Go look at google images or an obstetric book.

No gobs of money.
Decent wage due to 10-13 hour days 11 days a fortnight (then the added fun of 1-1 and a half hours of traffic to and from work each day)
Then all of said money goes off to pay off my house in tripple quick time.
Looking at going to one of the more peripheral hospitals next year, better education, better supervision, less of a 'bums on seats merry-go-round feeling.
