So the Bloodwrack Shrine comes with a Bloodwrack Medusa that can be modeled on her own base.
Is there a separate warscroll for her?

Also in the scroll builder, there is a point cost for the medusa and a cost for the shrine...do you take both of these together for the whole package?

Yes, she has her own warscroll, but you can't take the Shrine without the Medusa.

Basically, you can have a Bloodwrack Shrine (which comes with a Medusa), or you can just have a Bloodwrack Medusa (and make a Cauldron of Blood from the rest of the kit). The points for the Shrine includes its Medusa.

I don't think the warscroll for the foot slogging Medusa is on the GW website, but the one in the Dark Elf Compendium has the same rules, and you can get it on the AoS app.
