does the Reiver need to already be in grenade range to be able to use this stratagem ?

and secondly ... what happens if the target model retreats and this puts the grenade out of range where it was in range before?

According to my thinking, yes, you need to be in range to use the grenade (and it counts for your 1 grenade per turn). However, the grenade is used BEFORE reactions to stop overwatch and thus, the charged unit can only retreat after the grenade is thrown.
