So, I looked this up on lexacanium, and there's not a lot of info there - I'm curious if there's anything new in the newer fluff.

But since da emperah's golden boyz have been sent out to get stuck into the galaxy at large, they're probably going to start taking casualties.

How are new custodes created? Are they? Lexacanium seems to suggest that they might be based on some kind of emperor-class geneseed, but it might be even more deep than that. For example, apparently each custode was a unique work of art, created by the big E himself. If this is true, than, obviously, there can't be any more made.

So, can the imperium create fresh custodes?

Bonus question - could Primaris Custodes be a thing?

They can and they have. There were only a thousand of them left after the Battle for the Webway in M.30,but in M.40 they are back to their full strength of 10 000 Custodians.

6000 apparently the khorne boys killed 4K when he decided to solo terra.

setting aside how many exist "right now" custodes are created from the infant children of the nobility of Terra. no one's really sure how long it takes to create a custodes and what exactly goes into it (there is no geneseed. they're custom crafted)

The Codex suggests that there are new made. "Created by the Emperor himself" seems to imply he directly refined the technique to the letter, and when alive, oversaw the creation of each and every one. But the Custodes do make more, otherwise each Custodes alive would be 10k years old and that's not the case.

Each Custodes represents a unique handcrafted investment where they Space Marines are en-masse. Its highly unlikely Cawl ever had access to Custodes genetics and even then, Custodes are still superior in every way to the Primaris. They are also as yet uncorrupted, which is one benefit the Primaris promote over normal Marines, so the main advantages Primaris have is rendered pointless when applied to Custodes.
