My grandad wants to play a big battle with my 6mm minis. However, we both find epic a bit confusing. He struggles with too many rules and stats (and so do I)

We're looking for minimal stats and minimal book keeping. We just want lots of soldiers, tanks and carnage.

Take a look at 1page40k. I have never played it but the simplicity of all the rules for each faction being on one page and all the core rules being on another page might be ideal for it. How you'd fix it from a skirmish battalion game to more of a brigade formation game I don't know.

Seconded. Much less complicated than normal 40k, and the difference in model size shouldn't be a big deal.

. Psychic tests are unfluffy. Your longing for AV is understandable but misguided. Your chapter doesn't need a separate codex. Doctrines should go away. Being a "troop" means nothing. This has been a cranky service announcement. You may now resume your regularly scheduled arguing.

Another vote for Apocalypse, have played it with 6 mm armies and it works without a hitch. Very straightforward, less granular orders than Epic has and is much quicker to read when lots of details aren't desired.

Feels like a company level game, where Epic is a step above that.

Do you think it would work if I got rid of all the cards (I have a loathing of cards) and added in Cities of Death (soft and hard cover rules)?

I want a game where positioning etc is important because I intend to play lopsided scenarios such as: outnumbered but well dug in force needs to hold the fort against overwhelming odds etc.

Yes, though you might want to put something in the place of cards: currently you have characters leading detachments and generating cards while being weak fighters, so instead of cards they could offer rerolls or other buffs as an example. Currently the game incentivizes killing the enemy leaders, cutting down their card play, but that means you aren't killing the units that actually do the damage so there is lots of decision making going on.

Many cards are basically "reroll X" or "change order", so fixing that to your liking isn't hard. You have to rethink psychic powers though, as they are in the cards and pretty nasty if simply given to the psykers.

Adding cover is a breeze, it's a pretty abstract system.

Which version of epic are you finding confusing?

Epic Armageddon is the most streamlined and easy to play, with none of the complex rules of 1st or 2nd ed, but more complex and enjoyable than 3rd ed (epic 40,000).
