The Condemnor boltgun (and the relic version) say:
Each time and attack made with this weapon profile is allocated to a PSYKER model, that model's unit suffers D3 mortal wounds in addition to the normal damage.

I'm not exactually sure at which point an attack is allocated to a model, is it:
A) when the unit is selected as a target
B) when a hit is successfully scored
C) when a wound roll is successful
D) when a save is failed
E) when damage actually happens (after a shrugs or damage reduction etc)

The other question I have is about the new Hymns of Battle.
War Hymn and Chorus of Spiritual Fortitude both specify "Until the start of your next Command phase". None of the other hymns specifty this. Does this mean that their effects are perminent?
For example, Litany of Enduring Faith grants +1 to the Shield of Faith invul save (to a max of 4+) can this be stacked turn after turn?

It is in Step 3, the "Allocate Attack" part of the rules.

Page 18 of the PDF rules.

The Making attacks rules on P. 18 says "Attacks are made using ranged or melee weapons." and it has 5 steps:

1) Hit roll
2) Wound roll
3) Allocate attack
4)Saving Throw
5)Inflict damage

All Hymns last "while this hymn is inspiring". That means until your next command phase as noted on the datasheets for the models that are Priest.
