It says that "Chaos Models that may issue challenges must do so." This is part of the Eye of the G-ds special rule that only select models have. In order for a Chaos Model to be effected by this they must have the Eye of the G-ds special rule.

By the main rules only one challenge can occur in each combat instance. This means that only one challenge can be issued and its up to the controlling player to decide who issues that challenge.
