Example using The Masque for Chaos Demons in a 500pt game or using Ghazkull thraka for that matter in the same instance? Went through the rule book and all it says is they can be unique that's basically it.

The Ork Codex I cant find anything that says minimum requirements as well.

Any help is appreciated.

Yes, everyone plays Calgar in their 500 Point Games now... all 250 Points of him. The fact is, the points cost alone balances out. Would you rather have Ghaz or 37 More Boyz in a 500 point game?

The only codexes you will find points limits are old 3rd edition Codexes. Does this mean the point value has to equal 1500 or more, or does it just refer to a 1500 point game? Could you take him in a 1499 point game for example as often people are a few points under 1500 points in 1500 point games due to uneven point costs.

Can the special character's point cost count towards the point? Grimaldus is about 200 points. Technically, the Points value has to be 1500 or more, including the special character. A roster of 1499 cannot have a Character needing 1500. Technically.

Many 4th edition books have them, my tau come to mind.

It refers to a 1500 point game. AFAIK the characters points cost counts towards the total points of your army.
