Its a chimera tank kit.

He cut the back hatch off and made it slanted in two directions. Could do this with plastic card.
He hides the seem on the top of the model with a bit more plastic card with added rivets. You can see this directly behind the turret.

The turret has a bit added to the base, my guess more plastic card with rivets. Looks like it was done to bulk it up.
The flamer looks easy enough to make. Just got to do some chopping.

Hes used the packs to hide the lasgun ports that didn't get cut off.

i took the top part of the chimera kit and cut it where the lasgun-holes begin. flipped it and glued it onto the back. then i just glued the normal backdoor onto it
and used some of the spare sparts from the chimera kit and a little bit of green stuff to fill the gaps.
