Ambiguous:
Left Shoulder pad - my money is on Death Company or Sanguinary Guard

Right shoulder pad - appears to be the shoulder pad with the two small wing motifs on the corner, variations of which can be found in again, death company or sanguinary guard kits

Left hand - Based on the fact that the top of the shoulder pad is forward, implying that the arm is bent at the elbow, my guess would be a bolter hand from whatever kit you wanted to take it from. Could very easily be replicated with a new arm from one of the newer kits with an open hand attatched at the wrist.
