Online Co-operative Rules Writing Software/Web-design?

Hi there,

I am wondering if this type of software or web-interface-software for Co-operative document/rules writing exists already?
Basically the format would be this:
Each individual can vote on each rule ONCE (although they can change their vote at any time).
In order to vote you would have to create some sort of account (similar to a forum, making sure it is a real person doing the voting) while only allowing each account to vote once (positive or negative) on each rule.
If you vote negative a box pops-up and you must write some sort of comment in order to make a negative vote.
When a rule is changed all the votes and comments are erased and the process can begin again for the changed rule.

Here is what it might look like: (2 pictures below)

So, is there some sort of software or online document editing or website-design interface that can already do this type of thing?

(I am not particularly looking for something that is just 'similar' but more for something that is (or can be made to be) almost exactly as I have it in the pictures).

If so, could someone point me in the right direction?

Thanks!
