Cadian Chest, Upside down FW backpack + missile, possibly Khorne Berzerker head with the ears cut off, if only because it's the only real helmet with that face that I can remember, Elysian arms + pads I think, catachan command squad legs?

Greenstuff cloak
2 fire warrior backpacks cut in half and glued together to get both sides sloping (fire warrior pack only has one sloping side)
Berzerker helm heavily cutdown and greenstuffed
Arms are Elysian
Gun is cut down Kroot Rifle with Muzzle change.
