I don't think our individual physique is relevant, just our mentality. So the overweight, those in poor physical health and females can answer too.

So:

Yes chapter affiliation matters, some will look after your family for life if they provide a recruit, others don't care. Some chapters wont actually give you a choice, if you are worthy you are taken and if you are taken and refuse you are unworthy and you die, become a serf or worse a zygote.

It is an honour to serve as an astartes and they are sorely needed, all told you are better off accepting, if not for yourself. Most space marines die anonymous deaths on some alien world as a humble basic bolter marine. Few survive to be anything else and only a handful survive to retire, if they get to retire at all. Most chapters dont even have retirement as a concept. However anyone who is good enough to be an astartes at all is ultimately good enough to become a Captain if they live long enough. Being an astartes officer, or even a sergeant gives you a lot of authority, you can shape the destinies of entire planets and really make a difference with your life. And if you never get that far you are one of only a hundred or so astartes helping another Captain do same. There is no such thing as an insignificant astartes.

Now if you had a viable choice, join the astartes or join something else prestigious, such as becoming a Tech Priest or join the Inquisition then there might be alternatives. Anyone worthy to be an astartes is likely also worthy to become an apprentice Inquisitor, or to hold a Rogue Traders warrant. Those IMHO are the two of the most coveted jobs in the Imperium, and are preferable to becoming astartes.

I mean, bearing in mind when most recruits are recruited... if you asked 11 year old me, she'd have said yes without a shadow of a doubt. Doubly so if she were a denizen of the Imperium.

Like, have you SEEN high-def images of power armor? It's awesome.
