Do monstrous creatures have the instant death rule when making close quarter combat attacks vs. regular infantry (assuming that the strength of the monster does not equal or exceed twice the toughness of the defender)? If not, can armor saves be taken as per usual?

ID =/= AP2, but MCs have AP2.

Mc's have the smash rule, which causes all of their attacks to be ap2 and gives them the option to make a single strength 10 attack instead of their regular attacks.

They do not inherently have instant death, unless it is:
-listed seperately in their army list entry
-they are a psyker who has cast force prior to the combat phase
-They gain it from a piece of wargear, a relic, a warlord trait, or possibly a command benefit?
-Their strength is the double of their victim's Toughness.

Very small note - MC hammer of wrath attacks are not AP2. All other MC attacks are, though.
