Trying to understand how the revenant discipline is accessed.

Is this specific to only Yvraine and Yncarne or can the Warlock/Spiritseers/Fraser's access this discipline as well?

Ok. How many times can she cast Word of the Phoenix?

Data sheet says she can manifest two powers, but it doesn't state she can't cast the same one twice.

And if she is granted Herald of Ynnead she can manifest another.

So the potential is there to cast all three powers or one power three times?

No, not in Matched Play. From 'Psychic Focus' (main rulebook, pg. 215):

With the exception of Smite, each psychic power can be attempted only once per turn, rather than once per psyker per turn.

By my reading triple smite is not out of the question.

No, psychic powers are limited to once per turn, not once per phase. You can cast Ancestors' Grace normally in your own Psychic phase, but if you Soulburst in your Shooting phase you still cannot cast Ancestors' Grace again. You can, however, cast the same power twice in one battle round- once during your turn and once during your opponent's turn.

To be clear, the Psychic phase rules say that an individual psyker can only attempt to manifest each power once per turn. That's regardless of game type and would apply to Smite too. The Matched Play rules add in that you're limited to one attempt per power across your entire army, except Smite.

Meaning you can attempt to cast Smite until it goes off vs. casting over and over?

And is there a limitation upon psykers casting from inside vehicles?

Yes there is. From 'Transports' (pg. 183, main rulebook):

Embarked units cannot normally do anything or be affected in any way whilst they are embarked. Unless specifically stated, abilities that affect other units within a certain range have no effect whilst the unit that has the ability is embarked.
