Whilst it's left quite vague, basically some warp shockwaves from after the great rift opening triggers armies of orks and nids from octarius start to attack nearby systems. Here's the quote:

I don't think it'll go anywhere. It's part of GW's constant "things are bad but they're about to get EVEN WORSE!!!!!!!" obsession. Which they like pushing no matter what.

This is talked about a bit in the Tyranid book too. Ghazghull was able to save the Octarian capital world from Leviathan’s war-winning invasion and basically pushed the war back into a system-wide stalemate. That’s awful for the rest of the galaxy because Leviathan is stockpiling biomass which the Orks are kindly funnelling into the war from across the galaxy and then deploying that biomass elsewhere, and the constant war is the best thing ever for Orks because it is constantly culling the weak and ensuring the strongest survive, and by this point that’s led to some really scary Orks.
Good times if you’re a Tyranid Hive Ship or an Ork that knows how to survive. Bad news for everyone else...

Me thinks this is all just a lead up to inserting new fluff, armies, etc and then setting the story right back at the precipice of the never ending cliff. Then a decade from now, a new generation of players will lament at how the story never changes and blah, blah, blah and we will get an Emperor model as "proof" that it does change.
Sadly, I have to admit I'm not a fan of the story driving the game. I'd much rather see models (like Primarchs, the Emperor, Chaos Gods and Eldar Avatars) released without having to worry about the impact on 3rd class SciFi writing.

I'm guessing the great rift cut off their forces from each ohter then? I'm not sure why or how either side would be able or want to cut and run from probably the most bloody part of the war-torn Milky Way...

Neither is running, they are just spilling over. Leviathan is using the war as a factory to grow particularly nasty breeds of Heavy assault organisms which it can then send to other war zones. Essentially the Tyranids have discovered the mmorpg phenomenon known as “grinding”

This to me seems the way to AoS size increase Ork Boys in 40K when the inevitable Codex Update drops - hulking Skarboys could be the 40K equivalent of the AoS bigger than Black-Orc Orcs.
