Can't tell if this was an old model or some kind of kitbash.

I think that's a bit of a kitbash - the main body of the models seems to be based off the metal Chaos Lord that went OOP earlier this year, but with a chainaxe in place of a power axe (I think), and some other modifications.

It's a generic Chaos Lord kitbashed with a Berzerker axe. I have two of these dudes myself. He was released during 3rd.

Oh, and the helmet was taken from the plastic CSM sprue.

The 3rd ed Chaos Lord in this pose had a power-axe and plasma pistol.

There was a similar one released with the 3.5 Chaos Codex that had a raised powerfist holding a space marine helmet and Combi-melta. I recall that the powerfist and combi-melta were separate so it could be the same body as the 3rd ed Chaos Lord with separate bits for the fist and combi.

The chain-axe looks like it could have been from the plastic Khorne Berserker kit.
