Presumably, the Custodes would keep very quiet about any near-misses, but it doesn't mean there haven't been any.

I imagine Vandire came close- the Custodes granting an audience to the Matron Saints of the Sororitas seems like a fairly desperate move, and Vandire's forces were on Terra at the time.

Vandire was in control of Terra. He was ruling in the Emperor's name, as head of the Administratum and the Ecclesiarchy. He wasn't trying to kill the Emperor.

The Necron World Engine was making course for Terra.

...somehow... for some reason...

Everything about the World Engine was ridiculously stupid, but I guess they might theoretically have succeeded if they'd managed to pass off the idiot ball for a while.

Big chunks of the Inquisition are gaming this out while the rest try to stop them. Some Thorian inquisitors believe that the Emperor must be freed from his mortal shell to either ascend or be reincarnated. As far as how many actual attempts have been made, not really possible to tell

The Adeptus Custodes make attempts on the Emperor as something of a rite of passage and to help shore up defenses. The attempt described in the current codex looked it came pretty close to working.

Yes, the Custodes call these exercises the Blood Games. It would be hilarious if the Custodes acting the role of the assassin actually succeeded.

This must've happened when I was out of the hobby, I'm guessing it was a poorly thought out concept that never worked out.

What about the giant demon incursion on Terra that was occurring when the Astronomicon was offline. That was a pretty intense assault of the Throneworld and the Emperor.

I think a complicating factor here is that the Emperor can probably just turn around and nuke a would-be assassin if he really needs to, but then he drops his lighthouse beam and 10000 spaceships get lost in the warp.

So the Custodes aren't just there to protect his life but also to prevent him from getting distracted, because that's disastrous.

Anuvver fing - when they do sumfing, they try to make it look like somfink else to confuse everybody. When one of them wants to lord it over the uvvers, 'e says "I'm very speshul so'z you gotta worship me", or "I know summink wot you lot don't know, so yer better lissen good". Da funny fing is, arf of 'em believe it and da over arf don't, so 'e 'as to hit 'em all anyway or run fer it.
