That's what I've got for you based on what models you have. I don't know what the CSM are armed with, but I'm guessing that either some have special or heavy weapons, or you can bitz some up. Ideally, you should get a plasma gun & lascannon for one squad and a plasma gun for the other, with the rest of the CSM armed with bolters. The 8-CSM squad is made up of your Chaos Warriors, with their shield arms replaced with pistols if you can manage that.
Cunning Plan: The HQ start on the table, deployed behind terrain where possible, but with an eye toward using the Demon Prince's ikon in turn 2 and getting that speed lt into combat ASAP - he's got a 19-24" charge, making a turn 1 (if you go second) or turn 2 assault easy. The smaller CSM infiltrate into fire support positions, and the larger CSM squad infiltrates close, mostly to launch demons into combat and then follow them in. The DP can take tanks apart in combat, and the speed lt can melta them too in a pinch, to take the pressure of your CSM lascannon a bit.
This is pretty old school CHAOS in my mind. Brutal combat capability, very limited ranged fire power, and a weird collection of models
