Obviously it wouldn't be used just anytime because it would kill the Imperium prematurely.

Think like the ending of Gaunt's Ghosts: Honour Guard...all xenos and Chaos-touched Humans get killed, the Eye of Terror gets blown back, and malevolent warp entities get the shock of their life.

1. Travel to the throne room.
2. Kill me with your nemesis force weapon.
3. Hold the Imperium together through nine months of darkness until I am reborn.

I think it might be a big red button that does to Terra what Malcador did to Titan. Or maybe the self destruct button for the Webway portal under the Golden Throne.
Maybe it releases this really naughty monster that's trapped under Titan? Turns off some Stasis vaults on Macragge and the Rock?
One theory I did hear once though was that it brought back one of the last Primarchs. There was no explanation as to how, or why, but it kept a lot of children happy for a very long afternoon of being one of the oldest, yet shortest gamers in the room.

My guess is that it would either be the emporer commanding the grey knights to finish the necron device that was to tear the materium and immaterium apart so that they can no longer cross over into one another or that it has something to do with the story about the two inquisitors talking only hours after big E was throned about reviving him.
