I think that when it comes to where he's hit, you need to rely a bit on the narrative side (unless your group is really against that). If he doesn't describe it at all? Hit him in the torso. If he describes trying to ram his arm or leg? Throw him another -5 or -10 and allow him to target that limb. If he only fails by 1 or 2, let him hit the enemy in the torso. Head I'd throw a bit more of a - on, but if he hits make the target miss his next round unless he passes a hard or very hard Willpower test, on account of his massive concussion. It adds narrative and awesomeness. The narrative aspect exists for a reason, and it is an important one.
