Wondering if there are any companies that will match specific colours/shades into a pot from a sample and ship them out? Thanks

Oh, more specific than that? OK - well, most your home stores, paint stores and others have the ability to machine match paints. They can stick a sample under the scanner and generate a color formula for that paint sample. Cost when using over the shelf latex emulsion type paints from the big box stores is pretty cheap. However, those places are limited to the bases they have on the shelf (99% will be the latex emulsion paints) and those tend to not be ideal for miniature painting. Normally the smallest size will be a quart (which is a rather bit larger than a pot).

Paint specialty stores have the same capability and generally have several different options other than just the latex paint to mix the paints into. Various acrylics and enamels for painting furniture, architectural metal and other items are pretty common. Those work well enough for miniatures (especially the 0 VOC water based acrylics). Again - the smallest size is normally a quart.

The limit - unless mixing by hand from other paints - is the volume. It is really difficult to mix 1 ounce of paint from pigments and a base. Just a small drop of pigment will make a huge change in those volumes, so in order to actually be able to formulate them - you really need to be dealing with larger volumes (the larger the volume the more accurate the mix). You may end up being pleasantly surprised though and find that a quart of custom paint really doesn't cost more than a bottle of GW paint (here at least...UK may have other issues which jack the price up).

If you are really only looking for a pot worth - you will probably be best served by going to an art store and finding someone who has an eye for that sort of thing. I've done that in the past too (I am slightly red-brown color blind...so when matching things, I get a second opinion). They will often have someone at the store who is able to help or will be able to refer you to someone local.

Ok thanks, I was only aware of the first option and couldn't see Dulux being much good for model painting, and my google-fu was being rather weak.

I should note (or emphasize for posterities sake) that while those emulsion paints are not much use for miniatures - they are very useful for painting terrain. If you want your miniature bases to match your terrain...have them mix up a couple quarts that match the colors you use on your bases and then paint your terrain boards with those paints.

I have actually seen people paint boards using hobby paints...and well, you might as well just light a fire and throw your money into it.

I have quite a bit of experience at mixing colours by eye and can confirm it is a complete pita. However if you can provide a sample of the colour I would happily give it a go, and would only charge if successful and for the paint actually supplied.
