Are there any formations or detachment besides Mogroks Bossboyz that forces your warlord to be a model within it?

Also, if I take said formation as well as a CAD, will the mandatory warlord still be selected from it?

No formations I can think of, but a couple characters do specify it. Assurmen, and abbadon come to mind. That said, if they are in a allied detachment, they can't do it. Maybe the table catches fire at that point...

I'm pretty sure Abaddon requires you to take him as your Warlord.

Abaddon does require that you make him your Warlord. Asurmen I saw nothing that requires you to make him your Warlord instead of another character.

Asurmen had to be Warlord in the 6th ed codex. This was removed from his army list entry in the 7th edition codex.

Abaddon is only required to be your warlord if he is part of your primary detachment. It is written in his "mark of chaos acendant" rule.
