I once made a custom mission as part of a campaign I was in against my friend's CSM. It went rather well

I once wrote a very large and extensive mission.
It involved an apocalyptic mega-blast that started in the center of the map and scattered as the game went along. It represented a huge warpstorm, raging on the battlefield.
Being on the blast marker (which was represented with a circle of dice) meant a whole host of buffs and debuffs.
Debuffs included:
- Taking a Ld test (even when you auto-passed them). Failing it meant some serious debuffs.
- Flyers in it would have their facing randomly scattered before moving.
- Dangerous terrain for certain unit types
To counterbalance this units received a 3+ cover and 5+ invul that could never be improved. If they had their own saves, they could still take those. The did not receive this save if the attack was also in the warp storm.
As an extra, Psykers also got specific buffs and debuffs, making it harder to cast psychic powers, but in return making them more powerful.
We played it both as a regular KP match and as the standard maelstrom mission.
It was never meant to be a ultra-balanced competitive mission, just a fun "see what happens" mission. It was largely well received, although some people (mostly shooting armies) objected to it, feeling that they were disadvantaged too much.
That was kind of the whole point since I set out to make a mission that also tried to make assault a bit better. I was clear about this but I guess people don't like even playing fields.

I am running a narrative campaign right now. Each mission is custom to represent the setting/goals/etc. It is always tons of fun and genuinely adds some new energy and enthusiasm into the game.

Active armies, still collecting and painting First and greatest love - Orks, Orks, and more Orks largest pile of shame, so many tanks unassembled most complete and painted beautiful models, couldn't resist the swarm will consume all
Armies in disrepair: nothing new since 5th edition oh how I want to revive, but mostly old fantasy demons and some glorious Soul Grinders in need of love |

My friend and I once did an escort mission. Tau vs Tyranid. The Tau had to escort an ethereal from one side of the the table to the other. Meanwhile, The tyranid has a set of burrow holes (played like a trygons hole). All Tyranids started in Reserve, and had to roll on a die to see which hole they entered from. Playing tyranids, I played a bunch of genestealers to rip open the hull of the devilfish he was escorting him in, and thus I did successfully, winning the round.

I work at a FLGS and am running a themed tournament this weekend using custom scenarios based on the four major chaos gods. The tables will be set up with specific terrain to go with the missions as well. Currently I'm keeping the missions secret, but I'll post them afterwards and let you know how the realms of chaos treated the players.

I played a game with a friend and his Tryanids. Set up a fortified area with my Chaos and played about 1850 points and 2500 for him. And had him bring out his Nids in waves and I had to try and hold out.
We both had fun but will probably change it up in the future to be a bit more balanced.

Been playing a campaign between 3 players Dark Angels, Chaos and Grey knights all battling for a city, we drew up a map of a city then separated it into sectors on paper and then arranged terrain to look like said part of the map each team tried to capture and hold each sector eventually we had some games that had all 3 forces on one table. was really fun and challenging to keep troops supplied as the army moved forwards supplies dwindled if you lost sectors and your number of units suffered for it.
