This is the "Notbe," short for "The Thing that Should Not Be," sculpted by Tom Meier and cast in metal by 5th Field Fantasy Miniatures.

I love the inventiveness of this Pan's Labyrinth-style grotesque, and painted it up to be a Tzeentchian Blue Horror.

I like that a lot, it's a really nicely creepy mini too.

That's certainly an inventive sculpt and perfectly fits that Tzeentchian vibes like you said. Great work on those eyes; you have added some great detail.
