Well, your name for him is going to be affected by several things.
Such as, what chapter does he lead? A name like Mortarion isn't going to fit the Chapter master of the LifeGivers marines, if you follow me.
However, what I'm looking at doesn't really help me determine where he's from.
He's a Chapter Master with all Librarian powers. Ok. So we've got a psyker, and he'd have to be from a chapter that supports psykers to a new degree, to have a psyker for their chapter master.
Amulet of the Angels. The name is intriguing, but the effect is not, esp. given that the 5th ed codex gives all chapter master an Iron Halo, which has the same effect.
My biggest thing when I make a character is: how does he fit? I can't exactly make a Chaos lord stronger than Abaddon and claim he's the one really leading the crusade, since no one knows his name. BY the same token, I can't create a chapter master of the Ultramarines, and just pretend I've never heard of Marneus Calgar.
Now, where can your guy fit?
A quick search of Lexicanum shows me a chapter of Space Marines called the Angels Porphyr, of which little is known. In fact, the only knowledge certain about them that I can find is the fought in the Third War of Armageddon, and they resemble the Eagle Warriors.
So, let's fit your guy in. The reason so little is known is that the Angels Porphyr are regarded as overly strange by their battle-brethren, for their large number of psykers. The similarity in appearance with Eagle Warriors is a coincidence, or a mistake. An Eagle Warrior called out to the chapter, and someone mistook him for identifying himself.
With that up, Let me toss a few ideas:
First, let's up the Amulet of the Angels a tiny bit. Now not only does it provide the 4+ invulnerable save of an iron halo, but you may ignore the first Perils of the Warp wound you receive in a game.
Now, a name, as you originally asked for: Going with the angle I have presented, though you by no means have to take anything that I said, I would name him Alexandros Porphost, a name that strongly reflects his chapter's, but sufficient different to make him a stable character.
