I don't think they do it often but it is used as a last resort. It is also used if as said before the infestation is too large but it can also be used as a convineance. For instance if there are no civillians on a planet or not many and there is a large army of the opposite side they will destroy it with cyclone missles.

Exterminatus is very rarely used and even when it is other Inquisitors scrutinise just exactly WHY they used it. A world is a very valuable resource that cannot be squandered. To do so would be tantamount to heresy, Blasphemy and Treason.

Didn't Kryptman (ordo xenos) use exterminatus to create a firebrake in front of the tyranid menace? Wasn't he was then pretty much branded as a traitor for using such extreme measures?

It kinda sucks that the regular humans get the power to blow up planets. Surely the space elves, representatives of the dark gods or giant bug-like bio-mass could take a planet in one go. I can see a hive fleet going Unicron on some unsuspecting world.

Well i can see the Eldar being able to, but they just havent had a reason when they can just retreat.

The Nids pull exterminatus every time they win a planet, its useless for them to destroy a planet before they eat everything of use on it first. Planets taken by Nids become barren rocks, incapable for harboring life without any metals, water, or atmostphere left. pretty much the same thing as a virus bomb eh?

Not that often. In many cases they will leave a planet that they can't recover to the enemy in case they later get the armies/resources/whatever to do so. If they destroy a planet, they are never going to get it back.

Basically other races have devices that can wreak havoc with a planet, but not obliterate it. Last time baddies had stuff like that was with Abbadon and his Black Fortress's thingys.

And it's not like the Imperium has LOADS of these weapons capaple of killing a planet. They are one of the most valued possesions of the adepts of the Omnissiah. So valued that they probably would NEVER, EVER fire them. Luckily, inquisitors aren't so iffy.

Inquisitors shouldn't use Exterminatus unless it's the only option left. That said, there are a lot of planets in the Imperium; even when using Exterminatus in rare cases, it works out to a lot of cooked worlds.

Also, some Inquisitors (and Space Marine commanders, now that I think about it) are more impatient than others.

Depending on the type of Exterminatus the Planet could be left available for re-colinisation, see Thoth for example:
Thoth was a Desert World in the Segmentum Pacificus. It was a human world discovered in the Macharian Crusade and liberated from Chaos worshipers.

The Tallarn Desert Raiders 3rd regiment, especially chosen for the type of planet, led by Captain Al'rahem used the native N'go tribesmen and conducted a guerrilla campaign against the Chaos followers on the planet.

The Inquisition soon declared Thoth Amundi Heretica and had the world cleansed of all life with virus bombs. Al'Rahem campaigned for the N'go to be spared due to the aid they provided, and his requests were granted. The N'go were evacuated, along with genetic samples of the flora and fauna of the planet. After the virus bombing the N'go were returned to the planet.

Thoth was virus bombed during the Macharian Crusade, but is likely now to be habitable again as the effects of the virus bombs have probably worn off in the roughly 600 years since its destruction.

What about the Hive Fleets? Which would be more devastating to a planet?

Hive Fleets remove everything, Atmosphere, Water.. EVERYTHING. They even strip the rock and metals off the planet. There was a good article on the GW site a while back, about that stages of a Tyranid Invasion. Once they left, the planet was like 16% smaller

But there are different exterminatus methods. Some just incerate the atmosphere, some use cyclonic missile but would there be stuff that could rip apart the planet itself? Worse than hive fleets?

And is it at all possible for a Hive Fleet to miss something? So eventually life could come back?

Never been a recorded incident that left anything but a barran lifeless airless rock.

The Imperium actually cannot blow up planets very well. Abbadon can though

I do remember Night Haunter popping his Home Planet by Lance Striking the hell out of a weak point (for massive damage)
