Has anyone ever come up with a really cunning plan or even abuse of the rules? I myself once made a Pun-Pun character, and my group actually found this epic trick...

Those are all on the main d&d forums at wizards. My friend is notorious for making monsters.

He worked up a bard with 90+ diplomacy. With one or two skill checks he can make you his best friend when you are about to murder him in a fury.

Also had a shape shifting druid that got his Str up to 63. That char tore off a frost giants leg, shoved it up his own ass, then proceeded to throw him 120 feet up into a high cave ceiling with enough force to cause 20d6 dmg on impact with the roof.

The guy that was running the game never played again.

My personal favorite is casting Exploding runes in a tome over the course of a few days. 100 times. Maximized and Empowered.

900 points of damage that can not be resisted and no save allowed.

All you have to do is toss it at the person, get it within 5 ft and cast Amanuensis (level one spell) on the book.

When we were going up against the BBEG of one of our Mage games story arcs, we dropped a 7 round stasis grenade at his feet. It was about the only thing he couldn't counter in some way, the look on the DM's face was priceless. We spent the next seven rounds killing the mooks, then when the stasis ran out I pinned him to a swamp elemental.
