Rolling for your ammunition reserve and Weapon faults each turn similer to what is employed in necromunda.
Cause I'd love to see a squad of space marines have their bolters jam when a horde of gaunts is 10 feet away.......

I concur. If shooting was, you know, actually -decent- then perhaps this MIGHT have happened, and by might have I mean that the chances of this happening are the same as finding Ice cream in hell.
If all shooting weapons get 2 shots then yeah, sure, put ammo on the line. Otherwise, no. Just no.
And make all close combat armies slow and purposeful.

Its a good idea, but it takes the game in a direction in a direction i don't like, that is, complexity. Keeping the basic rules of the game simple makes the games more enjoyable and appealing, as it keeps the game from getting bogged down more in rules disputes. Its a reason I won't play Warhammer Fantasy.

Problem with "ammo/malfunction" rolls from a fluff stand point, in 40k, is reliability. i.e.: How often and likely? That is to say, the lasgun is possibly the most reliable and abundant firearm in the game, so how often would it jam/malfunction? Bolters held by Marines are of the best quality and the maintenance and car for them is second to none! So how often would they jam? Same thing for Eldar. Tau, well, it would be kind of fun to think about a bunch of top-new-tech guys hitting the field and finding the over complexity of their weapons, although gives them a good knock-down power, but that whenever they REALLY need it, it binds up or they're out of ammo! But then Tau don't really need the extra nick in the shins, nor might it really support the background.

The Space Marines, Lasguns, Tau, and Eldar all have weapons that I'd expect to perform well.
Space Marines take meticulous care of them, Lasguns are always described as the 'AK's' of their time, Eldar have used the things for who knows how long, and Tau meticulously test their designs (malfunctions amongst 'newer' weapons does make sense, although the Get's Hot rule seems to cover that possibility).
In a scenario, I could see these rules being applied to possibly a backwater PDF, or a rebellious IG army...could be a fun scenario actually. Would make some sense with Orks...but Ork shooting doesn't need any more nerfing than then majority BS2

I wasn't trying to pick on the Tau (I did say it may not really match with their fluff, and that the Eldar weapons WOULD be very reliable). But even as you pointed out, their newer weapons would possibly have issues, and since the Tau are the newest boys on the block (They have only been back in contact with the Imperium for about three centuries), as only six thousand years ago had they mastered simple tools and fire, most of their weapons are probably still very new, compared to all the other races, Orks excluded as every weapon they make is new and untested!
So I would agree that the Orks could use a fun jamming or horrible malfunction test! And considering their main roll, giving them a Gets Hot rule, modified so as to only neutralize their weapons and not kill the user, would be fun! Well obviously there would have to be a points or attribute compensation and the likely hood would have to be scaled appropriately.

Although, i have read 40k books where it says IG lasguns leach power in the cold and do have malfunctions and the same with bolters....

They would, but Daemons possessed weapons (that seems to be almost universal in the rare CSM focusing fluff) tend to negate such things. In fact, many times they are improved by it
Cultists on the other hand, would have this problem. After all, they are just a meat shield in the end. If they had rules for any race about weapon jams, it would most likely be them. Or Orks, i guess.

I would tend to disagree about that. But I find the general explanation for all things Chaos rather boring and pathetic. People complain that the new codex lacks fundamental fluff, but I think Chaos in general lacks fundamental foundation.

Ammo wise any laser weapon has electrical connections and by default contacts that get mucky or corroded and cease to work, any weapon can run out of ammunition if supply lines are cut and even the best maintained and reliable weapons fail....... Even AK's jam as occured during the famous north hollywood shootout, And to me, a master crafted weapon just means there has been a fancy case bolted on....

I think that every time a close combat unit moves, they should have to roll a d6. On a 1 they trip and are considered pinned.
You know. To represent the fact that sometimes people trip. It happens.
And on a roll of 1, close combat armies hit themselves in CC. You know. Sometimes you fumble and stab yourself.

I can just imagine a gretchin squabble or a nid scuffle breaking out or DE wyches becoming trapped in there own nets, guardsmen shooting themselves in the foot or even space arguing over who drank the most last night.....
