In a US address, the first line is the name, the second line is the street address, and the third line usually the city, state, and zip code. Occasionally a street address, if it's a big building or something, will take an additional line, listing the building or suite #. But for apartments, you usually just put it on the same line as the street name and number.
In that address, the "ct" is short for "court" (other common ones are Street, Drive, Avenue, Park, Circle, etc.). Falls Church is the city, and VA is the abbreviation for the state of Virginia.
Line 1: John Wargamer
Line 2: 34 Warhammer Ave. Apt. 40
Line 3: Hobbytown, NH 03103

What is the format elsewhere?
I have never been good with addresses and such, but it does appear that most countries use the same format.
I do suppose that many countries not having states could make it a bit confusing.
On a side note, I am pretty sure that Ct, indicates a cul-de-sac.

Here in the UK and Ireland, we have a slightly different system. Line 1: [Name] Line 2a: Apartment #/Floor Number etc Line 2b: # [Street Address] Line 3: City/Town/Village/Hamlet Line 4: County Line 5: Post Code Line 6: Country (useful if sending mail to other parts of the UK). Of course, Ireland is so small they they only have Post Codes for Dublin ( >_< )

Do you guys use cct?
Circuit? Or is it just one for one of our kooky planned cities where each road loops back on itself and turns the suburban development into a maze?
Canberra (national capital). Not so much a city, more of a series of roundabouts and mazes.

A court is a cul-de-sac in Australia.
We refer to the street as a cul de sac when describing it (position of house, where to turn etc) but it is called a court.

As an example that would be familiar to many brits.
Neighbours.
Ramsay St is not a street. It is a court/cul de sac.
You can't play street cricket in a regular street without being run over (still, it isn't that much safer in the Ramsay St/Ct either ).
N.B. There is no suburb called Erinsborough in Melbourne (with or without a Ramsay street).

Never heard of 'circuit' in the US.
I think courts are usually Cul de sacs but not always.

In the UK a court as address is usually a large apartment building. I've never heard of a 'circuit'.
An amusing comparison between US and Japanese addresses.

My memory might be a bit off, but for Russia it's reversed (at least, that's how I was given the address),
It was:
name
city
street
address/apt #
