Pentara IV was once a bountiful agri-world, teeming with myriad species. The jewel of the planet was a bustling city port of glittering silver.
However, during the Age of Apostasy many worlds in this sector were scanned for Holbidium ore, a rich fuel that powered the engines of the Imperium. Unfortunately, beneath its verdant plains Pentara had Holbidium in plenty.
Within three decades intensive strip mining had scarred the world beyond recognition.
The miners moved on when the ore ran dry; the Imperium's thirst is never sated. Chemical fuelled rain and harsh acidic clouds were the all that remained in the cruel sky.
The silver city that once had been a portal to eden was now pitted and stained. Only the hardy and the desperate can scratch a living there. And even then, it takes a brave soul.
For there are rumours that the mining awoke other, more foul things. For those that dare travel the old trails, the wasteland is filled with ghosts of long dead miners and shadows of darker alien threats.

I know how you feel; Names are not easy. But a contaminated world is! Think excessive mining/contamination like the example above, or perhaps a cataclysmic battle or volcano. Or maybe Daemonic infuence has shaped it into a noxious pit.
Other names? Bressus? Atlas? Sanctus? Chymaxia?
