If I purchase the 20 pt upgrade for carapace in my HQ, will that also include any extra models I add to the squad? I.e. bodyguards, master of the ordinance etc?

yes

Yes, it say "the entire squad (including any Regimental Advisers}" so yes they are included. It will not include independent characters deployed with the squad, however.

just if you took all the advisors and body guards it's a whole lot cheaper

It states this clearly in the codex. If it states "for the unit" you can take it to mean it is a single price.

Per model would be insane - a T3 4+ save model costing more than a terminator, potentially?

If it were 20 pts per model, it would say "20 points per model" the way it is listed in, for example, the Leman Russ Squadron entry under heavy support.
