Question: If I have a captain attached to a squad, and the squad is blown away save for the captain, does the captain make a leadership test to see if he flees?

Yes, as he is a part of the squad thet just lost 25% or greater casualties.

Furthermore he does not leave the unit until his movement phase(So any effects that occur on the unit, that is now dead, remain on the character).
