THAT gets my goat, especially since i could have won or at least contested the remaining objectives with about 2 more minutes of play.
The guy was a great guy, but not very focussed that day, i guess..
I'm sorry, but this is why I like turn timers. The scenario you present

A bamboozlement, no doubt.

Once had a game of 40k late night at my FLGS several other tables people playing D n D to our left more 40k to our right and Magic behind us. This guy kept loading very loud and disturbing metal on to his phone at max volume ever other turn to the point where even the D n D group had a few words with him by turn 5 i finally said man i dig the tunes but you are annoying everyone he then packed up his army and walked out of the store slamming the heavy wooden door behind him knocking a few blisters off the wall. Never playing him again haha

Only time I've had someone on the phone during a game was when a friend's kid was sick and his wife was talking to him. We stopped early so he could go take care of the kid.
Short of an emergency or a quick "I'll be home at ___, dear," please put your phone or iPod away while we play.
I'm glad I have a great group of players.

I don't mind texting during games. It's just the world we live in, you can't really expect someone not to text for two or three hours on a Saturday. Especially if s/he's got a significant other. Not responding to texts for three hours definitely didn't fly with my most recent ex.
Headphones I wouldn't be too happy about, though, nor having an actual conversation on the phone.

In doubles tournaments I frequently wear an earbud. 6-12 hours of solid nerd talk can be a bit much. Some have mentioned it's disrespectful, and I can't disagree. I like to think that as long as you pay attention and play the game it's all good. That being said I started doing it to calm myself while watching people cheat, it sets my teeth on edge but it's often not such a big deal that it REQUIRES being called out. I'm already being tabled and you want to draw line of sight from one Kan between a vehicle and a building but requiring you to bend a tape to touch a model and then fire the whole squadron yes, please proceed. /iPod selection Katy Perry "you're so gay"

I've seen people do it at magic the gathering - to block out all others.
TBH seems anal and arrogant.
If someone has earbuds in I assume that they will not be listening to me so there will be very little information going their way. I having unblocked auditory canals will be asking questions and recieving information. Yes its douchbaggery, but only a minscule amount compared to the level of disrespect that goes with that sort of behaviour.
Thankfully most of the people play with are sensible, we might have a bit of background muzak on but nothing at high levels.
(as a side rant I hate people who enforce their music on people). If I was on a bus and started preaching hellfire from the book of Erebus the thrice defiled* I'd be spurned. Yet no one says anything to the @sshats listening to stuff at 900dB. They get more deaf and have to have it up higher RAAAAAAAAAAAAAAAAAAAAAGE)

I awake from my sleep...shamble to the surface...cast my cold, metallic gaze upon the offenders...and then proptly harvest their souls for the Nightbringer.
Seriously, though, I would prefer someone *quietly* listen to a song with one ear and listen to me with the other than be giving me a TFG attitude the whole game. If they have to have another source of input to stay interested, so be it. But start texting your friends " lol im pwning a noob" and you will quickly find yourself at the business end of a Gauss Flayer...

If they are messing around on their phones, then I will excuse it if it's not all the time or if they give me a good reason. If they keep messing around on their phones throughout the game, then I will either ask them to put it away, tell them to GTFO, or start fudging rolls when they aren't looking.
If someone isnt paying attention again and again, I will just modify my dice rolls a little too. If they accuse me of cheating I tell them they cant prove it since they havent been paying attention. That usually works

I think it all depends on who/where you are playing. If it was in my FLGS against somone i didnt know very well then yeah i might find it a bit annoying (although i get the whole txt from the GF thing, not worth the hassle later on) but if its with people i know well i dont care at all. Most of the time we drink heavily when we play so it all decends in anarchy anyway and when people break the seal the game can get pretty fragmented.

I have no problem with it as long as they know what's going on, can hear me and don't have to say "pardon" more than once or twice during the game.

Friendly game: probably wouldn't care, but I likely wouln't play the person again. I like the idea of the game being social. 1 play rocking out to music in his headphones, the other player staring at said player doesn't seem to social.
Tournment: I'd polite ask them if they could remove them. If they flat out refuse. I'd roll with it, but think its odd. Tournment play can be pretty intense sometimes, even without either player being upset or confrontation. There's a lot more questions and clarifications. Having to check first to see if your opponent can hear you would get highly annoying.
Exception: Some people have hearding aids that look like headphones. If my opponent said that i'd be fine. I know a guy with sensory issues. He plays with 1 headphone in for background noise. For some reason it helps him focus and hear you better. Without it, there's to much stimulation and noise at the GT scene, and he cannot focus.

Also this.
Much the same could be said about an opponent constantly fiddling with their phone. Overly distracted players kill the human interaction component of wargaming and at that point I may as well be playing against myself.
Agree with all of this.

I don't think it would bother me as long as the person in question kept up with the flow of the game, paid attention, and kept me from having to ask him 2 or 3 times. If all of those criteria were met I'd have no problems.

It would bother me probably, I know the headphones would. Phone is another issue I have personal matter for which I am stuck on any noise my phone makes I have to check it.
So I can forgive phones ... for the most part.

Im not sure how wearing headphones would sit with me. I guess it would depend on how loud the music was. if they couldnt hear me when i asked them/said stuff, that would be where i would draw the line.
For me two things can really disrupt the game. Watching TV at the same time as the game is on. Real problem for some people i game with. Same peeps also have huge discussions while the game is on. It becomes arduous when the game slows because of discussions about the effectiveness of some vehicle or other. lame.
