Fatal combat. I want an axe hit to kill someone. At any level. I don't want to ever get hit by an arrow.

I don't like Hit Points. But I have yet to see a more balanced system.

And this only applies to roughly people sized targets. I don't want an axe or arrow to kill a dragon.

This is a gripe that cannot be fixed.

Sure it can, play Phoenix Command. One hit to any part of your torso from anything bigger than a butter knife pretty much equals being removed from combat (whether you die or are just rendered unconscious will depend on the dice gods).

On second thought, don't play it. I love it and respect it in a "this game has to exist" standpoint, but even I don't advocate trying to play it seriously.
