fists vs death guard (using C:SM but with 30k rites of war as opposed to chapter tactics IIRC), I think it was a 750 point game?
DG tac squad with captain opens up on a fist squad with bolters plus a plasma pistol, a plasma rifle and a plasma cannon. All 4 gets hot rolls are failed, resulting in 2 dead marines and a wounded captain. Returning bolter fire kills most of the squad due to some terrible armour save rolls. Poor guy

Edit: this was after a couple of rounds of him forgetting to keep the PC still so that it could fire

playing in a tournament a year or 2 ago with my decurion necrons against a 8 riptide list.

Coming into turns 3 I am looking a little dogged as I have only managed to take 4 wounds off the riptides in total.

My 6 man wraith squad get into a good position to charge and my 5 man scarab squad is a little further away. Wraiths charge first into a 3 man riptide unit and lose all but 1 guy but get in. The scarabs now charge in as well and make a lovely 10" charge.

Wraith completely miss everything. In return I let the tau player attack next and he puts 1 guy into scarabs and whiffs then 2 into the wraiths and kills it (I think he though the scarabs weren´t fearless. I have no idea). The scarabs then proceed to cause 6 wounds and he fails 3. Fails his leadership test and the scarabs run him down.

His disbelief at the occurrence and the laughing from the table next to us as they watch him remove 3 riptides with only scarabs in base contact felt good. It turned the game as I think he become concerned with scarabs and not the objectives that he was doing ok on.

I was playing toned down eldar vs my brother's orks.
My brother was trying to get the objective for holding twice as many markers as your opponent and needed to get a couple of my units off objectives so he splits off his warlord to charge a squad of ~5 guardians while his squad goes for some storm guardians.
The squad of boyz lose quite a few to overwatch (I had a couple of flamers) and proceed to fail a 5" charge.
The warboss (mega armour and lucky stikk) manages to take a wound from the guardians punching him then completely fails his to hit rolls and doesn't kill a single guardian. He then fails his leadership test and gets cut down by the guardians.

Blood Angels v. Guardsmen + DA, 6th ed.
2nd time using my terminator librarian (the first time he died turn 1 to a snap shooting lascannon dev. Go figure):

Pretty slow game in the opening turns.

my Librarian w/ telepathy failed to mind control a plasma cannon, then died the next turn to something.

I had no transports in my collection at this time so I was footslogging hard, not shooting much.

And then there was turn 2 deepstrike...

Full Assault squad lands bunched up surrounded by guard and DA, terminators with libby arrive where there is nothing but guard, an Executioner fresh from an NRA convention, and an armored sentinel.

shooting phase sees my 7 man terminator unit drop 80% of a guardman squad from behind their own cover, and I forget which psychic power my libby used (I might have failed it on 11).

The return fire:

-grenade launcher from guardsmen squad scatters wide and hits the DA biker sergeant. fails to wound but funny.

-Executioner right sponson=scatters wide from terminators, hitting the dead center of the assault squad and killing 8/10.

-Executioner main cannon=overheat, no damage

-Executioner left sponson= scatters wide from terminators, almost hitting a nearby guardsman squad.

-Sentinel plasma cannon=scatters into the same guardsmen squad, killing a rifle and the flamer . Much laughter from the peanut gallery (including the guard player).

-Assault squad pretty much wiped out by bolters, because evidently mere mortals can't be trusted with anything more complicated than a plank with a nail in it...

-one, ONE terminator killed by lasguns from the commisar-ed squad. The other 2/3 squads can't get a blow past the armor.

Not sure what happened with the DA plasma weapons, but I think the captain passed armor against the combi-plasma misfiring.

Result: Terminators drop in and do what they do best; bully the wea- I mean, sweep the light infantry.

They then stand there for a turn like Goku as bullets and bombs literally airbend around them (except for Kevin at the back, but no one liked him anyway!)

I still lost handsomely, but we didn't really care about the win conditions for the rest of this game. We more or less spent the next hour trying to process what just happened.
