Took a while to figure out how to replicate this one:
Upon posting a "Quick Reply" or a New Topic, I get the banner ad up in the title bar. But it has to be a new Quick Reply; the ads don't appear for "automatically appended posts."

I think opera uses a strange caching mechanism so if you load the site and see one banner before it logs you in, certain circumstances with opera might cause the cached banner (or cached ad code at least) to be loaded. I'll keep an eye on it but I've not been able to replicate properly yet.
