At my store they tend to add one or two pieces each game, but it's completely random so we often end up with some seriously intense pieces like the throne of skulls and mist-wreathed swamp in the same game.
So what are people's opinions on how much should be used? What about in tournaments?

I think for tournaments 5 pieces in plenty. You want to have a balance there.
Friendly games need to be more fluffy. There's nothing cooler than invading a town/swamp/castle/GY/etc. It's much better than using a bunch of random hills.

I really like the minor ones like Khemri quicksand and the scree slopes, because it makes it feel like an actual battle in the Old World, rather than a field. If I finish my move in a river of blood, why wouldn't they be afraid of me? hehe.

I just run it how it is setup in the book, whatever comes up gets thrown down.

People hate that in tournaments though, which is what I was really getting at.
Will these be ignored in tournament games? I certainly hope not, even though some of them can really mess you up

We've never played the default, where from what I can see you're likely to end up with a load magical weird crap all over the board. We do D6 + 4 pieces, but we only roll for a magical variant for one or two pieces - and sometimes none.
I think one or two items granting special rules can make a game more interesting, but if 5, 6 or even more pieces are magical then things start getting silly. I mean, are half the fences in the Old World really magical?

At tournaments in Brisbane (and seeemingly in most tourneys elsewhere in Oz) we have terrain pre-deployed.
We roll for it when you deploy in it, or when a model first moves into it.
