This'll be my take on Reclusiarch Grimaldus, Hero of Helsreach for 5th edition. C&C aswell as suggestions for points cost etc. welcome.

Reclusiarch Grimaldus

WS BS S T W I A Ld Sv
5 5 4 4 2 5 3(4) 10 3+

Wargear: Crozius of Mordred, Master-Crafted Plasma Pistol, Crusader Seals, Terminator honours (included in profile) Rosarius, frag and krak grenades

Crozius of Mordred: This Crozius Arcanum maul was wielded by Mordred, Grimaldus's master and tutor. It passed on to Grimaldus after Mordred died. This is a power weapon which lets the wielder reroll failed rolls to wound. In addition, Grimaldus always wounds enemies on a roll of 5+ unless he would normaly wound on a lower roll.

Special Rules: Indepentent Character, Litanies of Hate (as current), Unmatched Zeal (as current), Command Squad (as current), Hero of Helsreach, I have dug my grave in this place..., ...and I will either triumph or I will die!
Hero of Helsreach: Grimaldus leads three Cenobyte Servitors carrying the relics from Helsreach's Temple of the Emperor Ascendant (insert fluff from the current version). These servitors counts as normal Cenobyte Servitors and, in addition, as long as at least one of the servitors is alive, everyone in a squad that Grimaldus leads, joins or is attatched to except Grimaldus himself gains one extra A as long as Grimaldus is part of the squad.

I have dug my grave in this place...: Reclusiarch Grimaldus is subborn beyond comparison. He will not back down and will never surrender. When rolling for the distance moved due to Righteous Zeal, Grimaldus adds 2 to the roll. This is in addition to the effect of his servitors. In addition, Grimaldus and any squad he joins, leads or is attatched to ignores difficult terrain for the purpouses of moving or charging while Grimaldus is a part of the unit.

...and I will either triumph or I will die!: Grimaldus has taken many wounds, and has survived more than most Space Marines, including having an entire tempel collapsed on top of him. Grimaldus has the Eternal Warrior USR. In addition, if Grimaldus loses his final wound, roll a D6. On a roll of 4+, Grimaldus ignores the wound.

Wait so whenever his last wound is lost on a two plus he lives?!.... hmmmmm.... Why not, everytime the opponent takes his last wound they must reroll. E.g. a warboss scores three wounds successfully he must reroll one of them. Otherwise just give him 4 wounds...

The final rule might make him a bit too "necrony". How about just giving him FNP?

Points cost.... i'd say around 200 for this. It's a pretty nails character.

Well, the point was to make him really REALLY tough. I mean, he had an entire Cathedral collapse on him and he still lived (for comparison, look at what happened to the Blood Angels member that crawled out from under a building that collapsed on him...), but still, he's only got 2 wounds and what is basically a buffed FNP when he loses his last.
